18 AAC 90.535. Summary of treatment results

(a) If requested by the department, within 90 days after a permit issued under 18 AAC 90.500 - 18 AAC 90.540 expires, the permittee shall submit to the department a summary of the treatment results, including

(1) the dates of the treatment;

(2) the total amount of each pesticide used;

(3) an assessment of the success or failure of the treatment; and

(4) any observed effect on human health, safety, or welfare, animals, or the environment;

(5) monitoring results, if under this chapter the department required the permittee to conduct monitoring; and

(6) records required to be kept under 18 AAC 90.400 - 18 AAC 90.420.

(b) In addition to other sanctions provided by law, failure to submit a summary requested under (a) of this section is a basis for denial of a subsequent permit.
