Section 961. Required facility reports

7 AAC 53.961. Required facility reports

(a) In addition to any other reports that the department requires, a grantee shall submit a monthly residential child care facility report for each of its facilities for each calendar month during the period of the grant.

(b) A residential child care facility grantee shall file the report required by (a) of this section within 10 working days after the last day of each month.

(c) A monthly residential child care facility report must include

(1) the total number of beds in the facility and the number of beds that the department has purchased for each residential child care service;

(2) the total number of children enrolled in the facility at the end of the month, either as residents or for day treatment;

(3) the number of children enrolled in the facility at the end of the month, for whom the state has assumed responsibility under AS 47.10;

(4) the total number of children that were placed at the facility during the month;

(5) the number of children that the department placed at the facility during the month and the nature of the residential child care services for which the department placed each child;

(6) the number of children that persons or agencies other than the division placed at the facility during the month and the nature of the services for which each child was placed in the facility;

(7) the total number of children that the grantee discharged from the facility during the month;

(8) the name of each department-placed child that the grantee discharged from the facility during the month and the classification of the child as

(A) discharged to parent or relative;

(B) discharged to a foster home;

(C) discharged to a correctional facility; or

(D) discharged to another residential child care facility;

(E) runaway;

(F) discharged to self through emancipation;

(G) otherwise discharged;

(9) the total number of children placed by agencies and persons other than the department that were discharged during the month and the number of such children that were

(A) discharged to parent or relative;

(B) discharged to the department; or

(C) otherwise discharged; and

(10) a program report that gives the names and case numbers of all children that the department has placed in the facility and who are currently residing there, and a brief description of goals, objectives, accomplishments, and problems of the grantee in relation to those children, during the month.

(d) A grantee must submit a payment request for monthly grant payment by the first day of each month as required by 7 AAC 78.190(a) .
