8 AAC 45.025. Forms

(a) The board will, in its discretion and from time to time, prescribe and require the use of forms for the reporting of any information required by this chapter or by the Act.

(b) A person may obtain a list of prescribed and required forms by contacting the division at its offices in Juneau, Anchorage, or Fairbanks.

(c) A list or form referred to in this chapter may be obtained by writing to the State of Alaska, Workers' Compensation Division, P.O. Box 25512, Juneau, Alaska 99802-5512.
