Section 291. Family application

4 AAC 65.291. Family application

(a) A family that wishes to participate, or to continue to participate, in the program shall apply to a grantee in the community of the provider that the family has selected or intends to select. If there is no grantee in that community, the family may apply to the department's child care program office.

(b) Application, including a renewal application, must be made in a format prescribed by the department and submitted to the grantee or office, as appropriate, for review and approval.

(c) If the grantee or the office, as appropriate, determines that a family meets the eligibility requirements of 4 AAC 65.281, the family shall select a provider.

(d) At the time of application, a parent of the family shall read and sign a family responsibilities form, provided by the department, which includes the requirements of 4 AAC 65.381 and contains a statement that the person signing the form has read those requirements, understands them, and agrees to abide by them.

(e) If requested by the grantee or the office, a family shall provide documentation to support statements made about children, income, and other factors that would affect the family's program eligibility or family program benefit.
