3 AAC 11.010. Application for license

(a) A person seeking to engage in the business of making or offering to make deferred deposit advances under AS 06.50 must submit to the department

(1) a completed application for an initial or renewed license

(A) on a form provided by the department; and

(B) demonstrating the qualifications required under AS 06.50.020 ,

providing the information required under AS 06.50.030 , and including other information required by statute; the application must include information

(i) identifying the applicant, including the telephone number of the applicant;

(ii) describing the business operations of the applicant, including practices and procedures for making deferred deposit advances and for advertising and marketing; and

(iii) permitting access by the department to the applicant's credit history;

(2) a bond

(A) on a form provided by the department; and

(B) that meets the requirements of AS 06.50.040 ; and

(3) a check or money order, payable to the State of Alaska, in the amount of the application fee required under 3 AAC 11.020.

(b) A separate application form and fee under (a)(1) and (3) of this section is required for each physical location or Internet web site from which the person conducts the business. However, only one bond is required for a business with more than one location.
