AS 21.42.100. Alteration of Application.
- (a) If a policy of life or health insurance delivered in this state is reinstated or renewed, and the insured or the
beneficiary or assignee of the policy makes written request to the insurer for a copy of the application, if any, for
the reinstatement or renewal, the insurer shall, within 30 days after receipt of the request at its home office or at
one of its branch offices, deliver, or mail to the person making the request a copy of the application. In the case of
a request from a beneficiary, the time within which the insurer is required to furnish a copy of the application does
not begin to run until after receipt of evidence satisfactory to the insurer of the beneficiary's vested interest in
the policy or contract.
- (b) An alteration of a written application for a life or health insurance policy may not be made by a person other than
the applicant without the written consent of the applicant, except that insertions may be made by the insurer, for
administrative purposes only, in a manner that indicates clearly that the insertions are not to be ascribed to the
