AS 08.36.250. Renewal of Registration.
- (a) At least 60 days before expiration of a licensee's registration certificate, the department
shall mail a form for renewal of registration to each licensed dentist. A licensee who wishes to renew a license shall
complete the form and return it with the appropriate fee and evidence of continued professional competence as required
by the board. The department shall, as soon as practicable, issue a registration certificate
valid for a stated number of years. A licensee shall keep the registration certificate beside or attached to the
licensee's license. Failure to receive the registration form does not exempt a licensee from renewing registration.
- (b) When applying for license renewal, a dentist shall report to the board each instance during the prior registration
period in which the quality of the licensee's professional services was the subject of legal action.
