Section 100. Application for licensure by credentials

12 AAC 54.100. Application for licensure by credentials

An application for licensure by credentials as a physical therapist or a physical therapy assistant must be submitted to the department on a form prescribed by the board with

(1) all items required under 12 AAC 54.030(a) ;

(2) a report of the applicant's national physical therapy examination scores;

(3) verification of the applicant's licensure status sent directly to the department from each jurisdiction where the applicant holds or has ever held a license to practice physical therapy, one of which must indicate a current license in good standing;

(4) other evidence considered necessary by the board to determine if the requirements of the other jurisdiction were, at the date of issuance, substantially equal to the requirements of this state;

(5) verification of having

(A) been employed in physical therapy at least 60 hours within the 24 months immediately preceding the date the application is received; or

(B) passed the national physical therapy examination within the 24-month period immediately preceding the date the application is received.
