12 AAC 52.590. Prepackaging of drugs

For the purpose of supplying drugs to a prescribing practitioner, drugs shall be prepackaged in child-resistant containers under the direct supervision of a pharmacist and bear a label that contains

(1) the name, address, and telephone number of the pharmacy;

(2) the name, strength, and quantity of the drug;

(3) the lot number and expiration date of the drug, if not already contained on the unit-of-use or drug packaging;

(4) cautionary information required for patient safety and information; and

(5) the initials of the pharmacist.
