12 AAC 52.040. Change of pharmacy ownership

(a) An owner of a pharmacy shall return the facility license to the department within 30 days after issuance of a new license when ownership of the pharmacy is transferred.

(b) A new owner of a pharmacy shall apply for a new and separate facility license in accordance with 12 AAC 52.020.
