Section 70. License by examination

12 AAC 30.070. License by examination

(a) An applicant for a license by examination shall submit

(1) a completed application on a form provided by the department;

(2) the application and license fees established in 12 AAC 02.200;

(3) verification of the training or work experience required under AS 08.71.110 (a)(2) on a form provided by the department;

(4) verification of passing the applicable examination under AS 08.71.090 , within the five years immediately preceding the date of application, and maintaining the certification for spectacles or contact lenses, or both, sent directly to the department from the American Board of Opticianry or the National Contact Lens Examiners; and

(5) verification that the applicant has passed the applicable course described in 12 AAC 30.075.

(b) Repealed 2/26/2003.

(c) Repealed 1/5/2002.

(d) An applicant may pay the license fee at the time of application or at a later date. The applicant must pay the license fee, before a license will be issued.

(e) Repealed 1/5/2002.

(f) An applicant for a license by examination under this section may submit official transcripts documenting the degree requirements in AS 08.71.110 (b) as a substitute to the requirements of (a)(3) and (5) of this section.
