Section 645. Exclusions

7 AAC 43.645. Exclusions

(a) Tinted lenses are not a covered benefit unless medically necessary for a recipient.

(b) Contact lenses are not a covered benefit unless the recipient requires the lenses as a result of cataract surgery, aphakia, keratoconus, corneal degeneration, or rejection of an implant, or when other medical reasons exist.

(c) Prior authorization by the division is required for tinted lenses and contact lenses.
