Section 990. Definitions

7 AAC 34.990. Definitions

In this chapter, unless the context requires otherwise,

(1) "certified copy" means a copy of the original document that has been certified to be a true and correct copy of the original

(A) by the person who signed the original document;

(B) by a notary public or other person authorized to administer oaths under AS 09.63.010 ; or

(C) if a notary public or other person authorized to administer oaths under AS 09.63.010 is unavailable, in accordance with the provisions of AS 09.63.020 ;

(2) "debilitating medical condition" has the meaning given in AS 17.37.070 ;

(3) "department" means the Department of Health and Social Services;

(4) "minor" means a patient under the age of 18 who has not had the disabilities of minority removed under AS 09.55.590 ;

(5) "parent" means a biological or adoptive parent who retains the legal rights to make medical decisions regarding the care of the minor;

(6) "patient" has the meaning given in AS 17.37.070 ;

(7) "physician" has the meaning given in AS 17.37.070 ;

(8) "primary care-giver" has the meaning given in AS 17.37.070 ;

(9) "qualified debilitating medical condition" has the meaning given "debilitating medical condition" in AS 17.37.070 ;

(10) "qualified physician" has the meaning given "physician" in AS 17.37.070 ;

(11) "registry identification card" has the meaning given in AS 17.37.070 ;

(12) "written documentation" has the meaning given in AS 17.37.070 .
