Section 30. Application for license

7 AAC 09.030. Application for license

Application for a construction license for the construction of a health facility must be made to the department on a form provided by the department, and must include:

(1) the completed application form signed by an officer of the organization submitting the application who has authority to legally bind the organization;

(2) a letter signed by a review architect or engineer certifying that the design and construction drawings and specifications for the health facility meet the requirements of 7 AAC 12.437, 7 AAC 12.900, and other applicable state and local fire safety, electrical, building, plumbing, mechanical, and constructions codes; a review architect or engineer may issue a conditional certification when the design and construction drawings and specifications for the health facility indicate that the health facility does not meet codes or requirements under 7 AAC 12.900 (a) - (g), if a waiver has been requested under 7 AAC 12.900(h) ; if a waiver is not approved, the applicant shall provide to the department evidence of review and certification of revised design and construction drawings and specifications for the health facility; and

(3) a written statement from the certificate of need coordinator that the health facility does not require a certificate of need, or a copy the certificate of need issued for the construction of the health facility if a certificate of need is required by AS 18.07.031 .
