Section 660. Completion and filing

7 AAC 05.660. Completion and filing

The clerk of court shall be responsible for completing and filing a report for each such decree. The information necessary shall be determined by the State Registrar; and shall include such facts as are necessary to locate and identify the original certificate of birth, information necessary to establish a new certificate of birth, current address of the adoptive parents, and the action taken by the court. The report may also include, or be accompanied by, either a request from the adoptive parents that a new certificate of birth be established; or a request from the parents, the adopted person if of legal age, or the court that a new certificate not be established. The information necessary for the report shall be furnished by the petitioner for such adoption; additional information shall be furnished by the social or welfare agency or other person concerned. The clerk shall supply the additional required information relating to the action taken by the court. The method of monthly transmittal, transmittal forms, and any other information or reports required shall be in accordance with the instructions of the State Registrar in cooperation with the Alaska Court System.
