4 AAC 65.261. Records

A participating provider shall retain and make available for inspection during normal business hours upon request of the grantee or the department

(1) all required licenses and permits for operation;

(2) a copy of all provider policy information concerning hours of operation, rates, other routine child care charges, and the dates to which they apply;

(3) repealed 9/12/97;

(4) a copy of all facility monthly billing statements; and

(5) daily attendance records signed by a representative of the provider.
